TitleSub internal limiting membrane hemorrhage: causes and treatment illustrated by 3 cases

PurposeUsually, a preretinal hemorrhage is located between the retina and the posterior hyaloid. Rarely, the hemorrhage occurs between the inner limiting membrane (ILM) and the retinal nerve fibre layer. The aim of this presentation is to discuss the causes and treatment of a sub-ILM hemorrhage.
MethodsThree patients with a posterior segment hemorrhage are described in this presentation. In all three patients, a different etiology of the hemorrhage was noted: in one young patient, it was an idiopathic hemorrhage, the second patient was treated for chronic leukemia while in the third patient the hemorrhage was caused by Terson syndrome. All three patients underwent a pars plana vitrectomy at our department. During the surgery, an unusual location of the hemorrhage was observed. A more cystic appearance of the premacular hemorrhage was obvious. The roof of the cyst was carefully removed and embedded for electron-microscopic evaluation. In all three cases, the roof of the cyst was found to be ILM tissue, indicating that the hemorrhage occurred sub-ILM.
ResultsIn all three patients, there was an increase of visual acuity after vitrectomy. Causes and treatment of this unusual posterior segment hemorrhage will be discussed during the presentation.
