Title	Chemoreduction and local therapy for intraocular retinoblastoma
Abstract Nr.	A1022

Purpose	To evaluate the long term efficacy of systemic chemotherapy (chemoreduction) and/or local treatments as an alternative to external beam radiotherapy (EBRT) or enucleation for intraocular retinoblastoma (RB).
Methods	This prospective non-randomized study included all new patients diagnosed with Reese-Ellsworth (RE) groups 1 to 5 RB treated in our institution since Sept 1997. First-line chemotherapy included 6 cycles of carboplatin, etoposide,and vincristine. Local therapies included cryotherapy, transpupillary thermotherapy, and plaque radiotherapy. Exclusion criteria were unilateral RE group 5 RB and any RE group 5 RB with NVG.
Results	Among the 60 eyes in 41 patients, 18 eyes (30%) with RE group 5 underwent immediate enucleation. Forty-two eyes (70%) were initially managed with conservative treatments. After a mean follow-up of 40 months, tumor recurrence and/or new tumor was documented in 17 eyes (63%) with RE groups 1 to 4 (n=27) and in 13 eyes (87%) with RE group 5 (n=15). Tumor control was achieved in the 27 eyes (100%) with RE groups 1 to 4. Chemotherapy plus local therapy was unsuccessful in 10 eyes (66%) with RE group 5 eyes (n=15). Those eyes were subsequently managed with enucleation (n=8) and EBRT (n=2). The overall globe salvage rate was 100% for eyes with RE groups 1 to 4 and 47 % for eyes with RE group 5.
Conclusion	EBRT and enucleation can be safely avoided in managing RE groups 1 to 4 RB with chemotherapy and/or local treatments. Globe salvage rate for RE group 5 RB is lower.
