TitleShort term experience with "modern" trabeculectomy

PurposeBy differing from standardized trabeculectomy in the instrumentation used to produce the lamellar scleral flap and deep sclerostomy,modern” trabeculectomy aims to reduce the incidence and severity of early postop complications and then to improve postop IOP results.The purpose of our study was to evaluate safety and IOP reduction following this revisited approach.
MethodsRetrospective study including our 35 first consecutive procedures in 30 phakic patients (mean age: 61.3 years)with medically uncontrolled various glaucomas.Minimum follow-up was 3 months.All procedures were performed according to R.Hitchings and P.Khaw's protocol.1st month postop management included laser suturelysis, withdrawal of releasable sutures and 5-FU injections.Complete examination was carried out preoperatively and at day 1,7, at 1, 2 and 3 months and every 3 months thereafter.
ResultsMean follow-up was 5.1+/- 2.1 months.POAG and OHT were present in 27 /35 eyes.29% had previous trabeculectomy. Advanced glaucoma was present in 60%.Antimetabolites were used peroperatively in 20 eyes.Although transient and minor in all cases, 1st month postop complications were observed in 66% and decreased with training.Mean IOP decreased from 26+/-8.8mm Hg to 13.2+/-4.4mm Hg(p<0.05).Complete (target IOP reached without meds)and qualified (target IOP reached with meds)final success rates were respectively 69 % and 89%.Mean final visual acuity was not altered compared with preop level(p>0.05).
ConclusionModern” trabeculectomy is a relatively safe and effective procedure while decrease of early post op complications was associated with improving training.
