| Purpose | To investigate the clinical features, visual acuity outcomes, and adverse events in patients with retained lens fragments managed by pars plana vitrectomy (PPV). |
| Methods | We carried out a retrospective review of all patients who undergoing PPV for retained lens fragments at our institution between June 1997, and December 2002. |
| Results | The study included 52 eyes of 52 patients, with a median age of 74,5 years and a median follow-up after PPV of 16,6 months. The median interval between cataract surgery and PPV was 7 days. Among the 43 aphakic eyes, a posterior chamber intraocular lens was placed at the time of PPV in 36 eyes and an anterior chamber intraocular lens was placed in 2 eyes; 5 eyes remained aphakic. Visual acuity was >= 20/40 in 27 (52%) patients and <= 20/200 in 12 (23%) patients at last follow-up. Among the 25 patients with final vision < 20/40, the primary cause of visual impairment was glaucoma (6/25), history of retinal detachment (5/25), cystoid macular edema (4/25), preexisting ocular disease (3/25), corneal edema (1/25), and other causes (6/25). Timing of PPV was not significantly associated with better final visual acuity. Presence of an intraocular lens before PPV was associated with better final visual acuity but did not reach a statistical significant level (P = 0,09). |
| Conclusion | Dislocation of the nucleus into the vitreous during phakoemulsification is a serious complication of modern cataract surgery. Management requires secondary vitrectomy and lens implantation can be carried out in most of the cases during the same procedure. |
