TitleThe EVRS Macular hole Study
Abstract Nr.1043
PurposeTo determine the parameters influencing the outcome of surgery for idiopathic macular hole.
MethodsRetrospective multi-center European study of 4207 vitrectomy cases for idiopathic macular hole between June 2008 and June 2012.
ResultsOverall closure rate was 85.7% after a mean follow-up of more than 1 year. Higher closure rates were obtained in earlier macular hole stages. Success rate rises to 92% when surgery was performed in the first 4 weeks following symptoms. The use of dyes to visualize ILM showed not to improve nor limit visual outcome however anatomical outcome is better. The surgical outcome was not influenced by different dyes. ILM peeling was performed in 100% of cases. Different gastamponades showed no statistically significant difference in closure rate. Silicone oil tamponade was used in only 7% of cases. Different positioning regimens do not seem to influence the closure rate, but further study is warranted.
Considering functional results, BCVA was shown to improve 2 lines and keeps improving in time until 12 months postoperatively. Predictors for better visual outcome were closure rate, preoperative macular hole stage, preoperative visual acuity, combined phaco-vitrectomy surgery and the duration of macular hole.
ConclusionHighest success rates for idiopathic macular hole surgery are obtained if surgery is performed in the first 4 weeks following symptoms and when using dyes to visualize ILM. There are no differences regarding outcome between different types of gas tamponades. The influence of positioning on outcome needs further study.
