DoelEndophthalmitis is a severe postoperative complication. It is reported that intraocular antibiotic prophylaxis (IOABP) reduces the rate of post phacoemulsification endophthalmitis (PPE). Our purpose is to perform an epidemiological study of PPE cases at Erasme hospital (EH), to compare them to the literature in order to infer a potential benefit of the routine use of IOABP at EH.
MethodesMulticenter retrospective descriptive observational study of consecutive cases of PPE treated at EH from 2009 to 2013. We found records of PPE through INAMI codes (billing) corresponding to therapeutic intraocular injections of antibiotics.
ResultatenFrom 2009 to 2013, 56 PPE were treated in EH of which 42 were referred. For this period, the rate of PPE at EH is 0.269% and 0.0734% per year respectively for acute and chronic forms. In the group of patients having their cataract surgery performed at EH, the mean age is 74.50 years for both acute and chronic PPE. 28.57% of them were found to be diabetic. In the referred group, the mean age is 70.86 years and 22.5% were diabetic. The number of males and females is similar in the whole group. 21/56 cultures were positive. 13/14 gram positive cocci were sensitive to vancomycin. The final best corrected visual acuity in decimal fraction was more than 0.5 in 71% of PPE.
ConclusieBoth the incidence of PPE and the population with PPE at EH are similar to the literature. IOABP should be considered at EH, as a reduction in PPE from the same initial incidence is reported after its use. However, as there is no agreement on the benefit and safety of IOABP and considering the risks of emerging antibiotics resistances, we stay cautious about its routine use in our hospital.
