TitelOrbital Abscess after Mandibular Dental Extraction

DoelTo present a case of an orbital abscess after mandibular dental extraction.
MethodesA 32 year-old woman presented with rapidly progressive proptosis after mandibular dental extraction. She underwent a full ophthalmologic work-up, laboratory investigations and orbital imaging.
ResultatenA 32-year old woman was referred with proptosis of the left eye immediately after mandibular dental extraction. BCVA was counting fingers. There was extreme conjunctival chemosis and proptosis and near-complete limitation of ocular motility. IOP was 31 mmHg. Pupillary reflexes were normal. Laboratory examination was suggestive for infection. Fundoscopy revealed venous extravasation but no optic disc oedema. Rapidly progressive proptosis and decreased vision suggested an orbital abscess. CT showed diffuse extra- and intraconal cellulitis, with extraconal abscess formation in the inferior rectus region. The latter was successfully treated with surgical drainage and intravenous antibiotics.
ConclusieOrbital abscess formation after mandibular dental extraction is a rare but severe complication. The presence of periocular inflammation after dental extraction should alert the clinician. Prompt recognition of symptoms can lead to early diagnosis, which is vital to preserve the functional integrity of the eye and to prevent potential life-threatening complications. The mandibular origin of the infection makes this case unique.
