Doel | To present the first results of Descemet membrane endothelial keratoplasty (DMEK) at our centre, University Hospital Leuven. |
Methodes | This single centre retrospective study included patients with endothelial decompensation who underwent DMEK surgery between September 2014 and June 2015. All the surgeries were performed by two surgeons (BF and MTH). We measured pre- and postoperatively the best-corrected visual acuity (BCVA) in logMAR, pachymetry and corneal endothelial cell density (ECD). |
Resultaten | Thirty nine eyes of 37 patients were included for analysis. All patients but one were pseudophakic. The analysis at 6 months is based on 10 eyes. We needed to do a rebubbling in 5 of the 39 eyes (13%). Four patients (10%) needed a second operation because of graft failure, we excluded those patients for further analysis. The mean BCVA (logMAR) was 0.58 ± 0.3, 0.25 ± 0.27 and 0.18 ± 0.17 preoperatively, after 3 and 6 months respectively (p:0.0002, p:0.000013). The mean decrease in pachymetry after 3 and 6 months was 113 µm ± 69 and 92 µm ± 20 (p: 9.10-7, p:0.0005). The mean reduction in ECD after transplantation was 1108 ± 614 and 1077 ± 386 after 3 and 6 months respectively (p:8.10-6, p: 0.0001). |
Conclusie | DMEK is an efficient and safe surgery for endothelial decompensation. The rate of secondary surgery and rebubbling is 10 and 13%. These complications will likely diminish with increasing surgical expertise. Most eyes have a good postoperative VA (mean 0.18 after 6 months). These results are even better when excluding patients with other ocular problems. |
