DoelTo report a late development with a long-term progression of a bilateral macular chorioretinal atrophy in a patient after an accidental electrical injury
MethodesA 40-year-old man presented a bilateral loss of vision soon after severe full body electrical injury (11 000 Volts). The follow-up during seven years includes full ophthalmological work up with Visual Field (VF), Fluoangiography(FA), macular OCT, Visual Evoked Potential(VEP), full field and multifocal ERG.
ResultatenAt presentation, BCVA was 8/10 RE and 2/10 LE (Snellen chart). The fundus was normal RE and shows a foveal cyst LE.
At 19 months follow-up, the BCVA was progressively reduced to 2/10 RE and 1/10 LE. Ophthalmoscopy and macular SD-OCT were reported to be unremarkable in both eyes. But, visual field presented severe central defects in both eyes. VEP suggests a normal response of the optic nerve but a reduction of macular response.
At 5 years follow up, BCVA remained to 2/10 RE and less than 1/20 LE. A bilateral pigmented macular chorioretinal atrophy developed, with a more extensive lesion in LE. Macular SD-OCT confirms atrophic scars nearly the fovea in both eyes in association with reduced response at multifocal ERG.
At 7 years follow up, visual acuity continued to deteriorate to 1/10 RE and less than 1/20 LE. Fundus and macular SD-OCT showed a progression of pigmented chorioretinal atrophy.

ConclusieOur clinical case presented a bilateral macular chorioretinal atrophy after electrical injury. Onset of loss of vision appeared soon after the injury, but macular scars developed about 4 years later, and visual loss continue to progress until 7 years of follow-up.
