TitleElevated intraocular pressure due to an iris ring melanoma: a case report
PurposeWe describe a case of a 38-year-old male patient presenting an elevated intraocular pressure (IOP) due to a previously undiagnosed iris ring melanoma.
MethodsA 38-year-old male patient was referred to our department with an elevated IOP in the left eye. He complained about recurrent redness, ocular pain and a blurry vision since a few months. Best corrected visual acuity (BCVA, decimal) was 1.0 and the IOP peaked at 30 mmHg. BCVA in the right eye was 1.0 and the IOP was 11 mmHg. The slit-lamp examination as well as the anterior segment optical coherence tomography (AS-OCT) and ultrasound biomicroscopy (UBM) revealed a large melanotic vascularized and nodular iris mass from 3 to 9 o'clock with an almost 360 degrees extension into the anterior chamber angle.
ResultsHe was diagnosed with an iris ring melanoma and was treated externally with two sessions of proton beam radiation therapy to treat the tumor as well as cyclophotocoagulation to reduce the IOP. Four months postoperatively IOP was stabilized at 18 mmHg with two glaucoma medications and BCVA was 0.4. Slit lamp examination showed signs of tumor regression with iris atrophy.
ConclusionIris ring melanomas are a rare type of uveal melanomas and quite difficult to diagnose. They should be considered within the differential diagnosis of nodular vascularized iris masses, especially in younger patients with elevated IOP. Proton beam radiation therapy is an effective therapeutic option, but regular systemic screenings, especially of the liver are recommended, due to high metastasis rates.
