TitelIs PRK better than LASIK for extreme myopia?

DoelTo compare one-year outcomes of extremely myopic patients between PRK and LASIK.
MethodesWe present a retrospective, consecutive case series and chart review of 219 eyes with more than -10.00D spherical equivalent (SE) preoperatively that underwent laser refractive surgery using Allegretto Wavelight laser. 182 eyes were treated via trans epithelial PRK and 54 were treated via LASIK. One year postoperative uncorrected visual acuity (UCVA), best spectacle corrected visual acuity (BSCVA), manifest refraction predictability and complications were evaluated and compared.
ResultatenMean preoperative SE was -11,53 and -11,26 in the PRK and Lasik group respectively. 38% (69/182) PRK patients showed UCVA ≥20/20 compared to 24% (13/54) LASIK patients. 60% (109/182) PRK patients had no change in BSCVA compared to 67% (36/54) LASIK patients. In both groups patients lost one line in BSCVA (16% in PRP versus 13% in LASIK group). 66% (120/182) PRK patients showed postoperative SE +/- 0.50D compared to 61% (33/54) of LASIK patients. Mild complications occured such as temporary loss of vision due to punctate keratopathy (2% (4/182) PRK patients compared to 22% (12/54) LASIK patients) and early haze formation (3% (5/182) PRK patients compared to 0% (0/54) LASIK patients). Only the higher amount of punctata keratopathy in the LASIK group was statistically significant (P<0,05).
ConclusieIn extreme myopia, UCVA was better after PRK than LASIK. This result was statistically significant. PRK for extreme myopia appears to be better than LASIK with potentially higher safety.
