TitlePlaque Radiotherapy and adjuvant delimiting transpupillary thermotherapy for choroidal melanoma

PurposeTo evaluate tumor control and treatment complications following Iodine 125 plaque radiotherapy (IPR) combined with delimiting transpupillary thermotherapy (DTTT) for newly diagnosed choroidal melanoma.
MethodsDesign : Prospective randomized case series
Participants : Of the 221 patients treated with a median radiotherapy dose to tumor apex of 90 Gy, 91 patients (41%) were managed with IPR alone (control group) and 130 (59%) received adjuvant DTTT (treatment group).
Methods : For those patients randomized to DTTT, one session of TTT was provided at the time of plaque removal in 99 cases (76%) and 31 cases (24%) after complete resorption of the subretinal fluid (mean interval of 11 months).
ResultsAfter a mean follow-up of 54 months, tumor recurrence was documented in 5 patients (5%) in the control group, and in 3 (2%) in the DTTT group. Treatment-induced complications included retinopathy in 55%, papillopathy in 16%, vitreous hemorrhage in 29%, neovascular glaucoma (NVG) in 17% and late enucleation in 9% in the control group. In the DTTT group, retinopathy was documented in 66%, papillopathy in 25%, vitreous hemorrhage in 18%, NVG in 7% and late enucleation in 4%. The incidence of vitreous hemorrhage, NVG, and late enucleation was statistically higher in the control group. The higher rate of tumor recurrence in the control group was not statistically significant.
ConclusionCombined DTTT and plaque radiotherapy provides excellent local tumor control and a statistically lower risk for vitreous hemorrhage, NVG, and secondary enucleation.
