Title

Microincision phacoemulsification using microtip and « D »Cartridge: clinical study.

Purpose	To determine safety for performing microincision phacoemulsification using the microtip and whether a full-sized single-piece acrylic intraocular lens (Acrysof IQ IOL) can be safely implanted through a 2.2 mm incision using the new D cartridge (Alcon Labs).
Methods	100 consecutive patients had cataract surgery by microcoaxial phacoemulsification with use of microtip (Kelman miniflare ABS 30° microtip and ultrasleeve 0,9 mm : Alcon Surgical) through a 2.2 mm incision using the D cartridge. Anterior chamber stability, incision competency, safety of IOL implantation, intraoperative complications, efficiency, and the learning curve were evaluated. Intraoperative results and the first postoperative day were evaluated concerning wound appeareance, Seidel and postoperative complications were also analyzed.
Results	Safety of IOL implantation, anterior chamber stability and incision competency were consistently excellent. A high-power IOL (28 Diopters) insertion difficulty was present. Efficiency and safety were equivalent to those in traditional tips and B or C cartridges. The learning curve with the new cartridge and microtip was minimal, although IOL implantation for high power IOLs was remarkably difficult. The incision was regarded during and immediatelly after the procedure, as well as in the D1 ; no signs of burning, Seidel or other wound problems were seen. There were no postoperative complications.
Conclusion	The clinical results in a prospective series of 100 patients confirmed the efficacy and safety of microcoaxial phacoemulsification using the microtip and the new D cartridge for implantation of the Acrysof IQ IOL. Fluidic advantages, competent incisions, and the new cartridge provide another approach to achieving less-invasive, high-quality cataract surgery.
