TitleBlackening of Choroïdal Haemangioma after Tantalum clip surgery

PurposeA red-orange surface colour on fundus examination is one of the key elements while diagnosing a circumscribed choroïdal haemangioma. Blackening of this benign choroïdal tumour has been described in one case, after photodynamic therapy.
MethodsCase report: A 53-year old gentleman was diagnosed having a circumscribed choroïdal haemangioma with secondary retinal detachment on the basis of typical findings on fundus examination, ICG angiography and ultrasonography. External proton beam irradiation was planned, preceeded by tantalum clip surgery. On the first day after an uneventful tantalum clip surgery, the surface colour of the tumour had changed from red-orange into dark gray, typical for choroïdal melanoma. Proton beam irradiation with 20 Gy was nevertheless performed as planned. Control fundus examination one month later revealed a surface colour turned red again.
ConclusionSurgical manipulation of the eye can cause blackening of choroïdal haemangioma, taking away one of its key diagnostic elements.
