TitleClinical evaluation of the new dynamic rebound tonometer ICARE®.

PurposeTo evaluate prospectively the intra- and interobserver variability of IOP measurements with the rebound tonometer Icare® and their correlations with Goldmann applanation tonometry (GAT) and central corneal thickness (CCT).
Methods3 consecutive IOP measurements were performed in a masked fashion by 3 different observers with Icare® in 54 normal subjects and 50 hypertensive/glaucomatous patients (GL) (IOP>22 mmHg with GAT). GAT was done by the same clinician. In all the 50 normals and the first 25 GL, Icare® measurements were followed by GAT. In the other 25 GL, the reverse sequence was respected with GAT first and then Icare® after 20 minutes break to exclude tonographic effect. CCT measurements were performed with Pachette™ ultrasonic pachymetry.
ResultsIntraobserver Icare® variability was comparable for each observer in the 2 groups. Mean coefficient of interobserver variability was 6.4% in normals and 6.6% in GL. Icare® slightly overestimated IOP comparatively with GAT in normals (range: 1.5 to 2.2 mmHg),proportional to GAT measurements. No significant difference was found between Icare® and GAT in GL. IOP measurements were not influenced by the followed sequence. Icare® correlations with CCT were only significant in right eyes.
ConclusionIntra-and interobserver variability of Icare® measurements is low and comparable with GAT. Icare® measurements are in good concordance with GAT. Due to discrepancy between Icare® and GAT in normals, it is advisable to control Icare® values >24 mmHg with GAT. Icare® measurements appeared to be influenced by CCT.
