TitelIntroduction: when is uveitis of autoimmune origin ?
DoelNon infectious uveitis are generally considered to be of autoimmune origin. However, experimental and clinical evidences suggest that autoinflammatory mechanisms are also involved. Autoimmunity and autoinflammation are respectively due to a dysfunction of the adaptative and the innate immune system. The function of those two systems is to discriminate and eliminate non self elements. The innate immune system came first in the evolution and use receptors that recognise conserved pathogen patterns. The adapative immune system relies on the generation of antigen specific lymphocytes clones. Autoinflammation occurs when the innate immune system is abnormally activated and autoimmunity if autoreactive lymphocytes mount an immune response against auto-antigen. Endotoxin induces uveitis and experimental autoimmune uveitis represent elegant laboratory illustration of the involvement of autoinflammation and autoimmunity in the development of uveitis. Similarly, abnormal function of pathogen associated receptors as well as activation of autoreactive lymphocytes have been described in patients with non infectious uveitis. It is thus likely that non infectious uveitis represent a heterogeneous group of disease. Each subtypes might differentially involved autoinflammatory and autoimmune mechanisms. A better understanding of this interplay will improve the management of our patients
