| Doel | To report the rapid apparition of stage 4 diffuse lamellar keratitis (DLK) and its spontaneous resolution within 3 months. Stage 4 DLK is the most severe form of the disease with an incidence of approximately 1 in 5000. It result habitually in a variable degree of visual acuity and quality loss. |
| Methodes | A 24-year-old man underwent bilateral uncomplicated laser in situ keratomileusis (LASIK) for myopia. Postoperative course in the right eye was uneventful; however, in the left eye, stage 2 diffuse lamellar keratitis was present at day 1 and developed till grade 4 at day 2 under oral and topical corticotherapy. |
| Resultaten | DLK was associated with stromal melting, deep flap folds, hyperopic shift, irregular astigmatism, and 4 lines loss of visual acuity. The rapid evolution from grade 2 till grade 4 could not allow us to lift the flap and irrigate the interface. The only therapeutic solution was conservative. The condition improved spontaneously and 3 months later, slit-lamp microscopy showed resolution of the folds and haze with subsequent improvement of visual acuity. There was a progressive improvement on refractive maps concerning corneal thickness, curvature, and regularity. |
| Conclusie | This spontaneous positive evolution indicates that observation might be a therapeutic option for stage 4 DLK. |
