DoelTo retrospectively study the outcome after slitlamp-delivered laser photocoagulation for threshold ROP using general anesthesia and topical anesthesia with sedation.
MethodesThirty-eight eyes of 19 premature babies with zone II threshold ROP were treated with slitlamp-delivered 532 nm laser photocoagulation using a contact lens. The infants were swaddled and held in lateral decubitus. Tight scatter photocoagulation was applied to the avascular retina from the vascular ridge to the ora serrata without scleral indentation. Ten babies were treated under general anesthesia in the operating room (group 1). Nine babies were treated in the neonatal intensive care unit using topical anesthesia, sedation with chloral hydrate, oral sucrose and a dummy (group 2); 2 infants received bilateral supplemental treatment because of disease progression.
ResultatenROP regressed in all eyes. In group 1 after surgery increased respiratory support was necessary in 5 infants and analgesia was administered in 7 cases; the time to regain full enteral nutrition was 27,4 hours. In group 2 neither increased respiratory support nor postoperative analgesia was necessary after treatment; in the first 2 cases full enteral nutrition was re-established after 15 hours, 1 baby received total parenteral nutrition, in the 6 most recently treated cases enteral nutrition was not interrupted.
ConclusieThreshold ROP can safely and effectively be treated using slitlamp-delivered laser photocoagulation even under topical anesthesia. Omitting scleral indentation may be one of the reasons for the smooth postoperative recovery.
