Doel | To evaluate tumor control and treatment complications following Iodine 125 plaque radiotherapy (IPR) for large choroidal melanoma. |
Methodes | Prospective observational case series. 77 patients with uveal melanoma with thickness greater than 8 mm. All patients underwent IPR (median dose to tumor apex, 90 Gy) according to a standard protocol. Follow-up ocular and systemic evaluations were performed every 6 months. |
Resultaten | The median tumor diameter and thickness were 15.7 mm (range, 10 to 22) and 9.4 mm (range, 8 to 15). After a mean follow-up of 43 months, local tumor recurrence was clinically documented in one patient (1%) and neovascular glaucoma in 18 patients (23%). Late enucleation was performed in 6 cases (8 %) after a mean interval of 35 months after IPR. Kaplan-Meier estimate for neovascular glaucoma was 4% at 1 year, 30 % at 3 years and 40% at 5 years, and was 0% at 1 year, 8 % at 3 years and 20% at 5 years for late enucleation. Systemic metastatic disease was documented in 17 patients (22%) after a mean interval of 26 months after IPR. The Kaplan Meier estimate for melanoma-specific death was 30% at 5 years . |
Conclusie | IPR seems to be a reasonable alternative to primary enucleation by providing a fair chance of preserving the eye of those patients with large uveal melanomas who have a poor systemic prognosis with short survival. This information can be used to counsel patients with large uveal melanoma traditionally managed with primary enucleation. |
