Doel | In general, children develop suppression quickly whenever a change in the binocular situation occurs. This is why childhood diplopia is a relatively rare condition. The causes of childhood diplopia vary widely, from well known causes such as dissociation following occlusion treatment to different forms of paretic strabismus. It is the purpose of this paper to discuss the orthoptic approach of these patients. |
Methodes | We present a case history of an 8-year old girl with diplopia of recent onset. We discuss the orthoptic examination of the primary position, of the ocular motility and convergence and of the sensory aspects. At every step of the examination, the selection of the test and the implications of the results are highlighted. |
Conclusie | We can conclude that: • a child complaining of diplopia should be taken seriously and should be examined with the same urgency as adult diplopia patients; • a thorough orthoptic examination may provide information to obtain a correct diagnosis and allows to follow the possible spontaneous evolution; • sensory examination is important to understand the nature of the diplopia and will give important information regarding the binocular prognosis; • if fragile fusion or diminishing suppression are demonstrated, urgent treatment is recommended: prism’s and/or rapid surgery. |
