DoelCollagen cross-linking using ultraviolet-A (UVA) irradiation combined with
the photosensitizer riboflavine is a new technique for stabizing progressive
keratoconus.
MethodesAfter abrasion of the corneal epithelium, riboflavine (vit B 2) drops are applied on the cornea until impregnation. Subsequently the eye is exposed to UVA light(370nm/3mW/cm_) for 30 minutes at a distance of 1-cm. A bandage contactlens is applied to diminish post-op inflammation and pain. Follow-up of the stabilization of the keratoconus is done by regular examinations of the corneal topography. Once stabilization is demonstrated, the residual refractive error (myopia and irregular astigmatism) can be treated by means of topography-guided surface PRK or by means of the implantation of a toric Artisan phakic IOL.
ResultatenSome comments will be given as to the way the procedure can be performed,
followed by a review of the results obtained by cross-linking performed in
other Ophthalmological centers (Clinical study phase-I, 1999).
ConclusieCollagen cross-linking has shown to increase effectively the biochemical and biomechanical rigidity of the cornea and to stop or even to reverse the
progression of keratoconus. Long-term results are necessary to evaluate the duration of the stiffening effect and to exclude long-term side effects.
A European multi-center clinical study phase-II starts in October 2005, in which our center will participate.
