| Doel | To determine the diagnostic accuracy rate and complication of needle aspiration biopsy (NAB) for selected orbital lesions. |
| Methodes | Orbital biopsies with a 19 or 23-gauge needle were used in 17 patients presenting with an infiltrative orbital lesion in which clinical and radiologic evaluation was not contributory. NAB was performed through a trans-palpebral approach without general or local anesthesia. Cytocentrifuge smears were prepared from the aspirated material. |
| Resultaten | In 16 cases (94%) positive cytologic identification was obtained by NAB. In one case (6%) incisional biopsy was mandatory to establish the histopathologic diagnosis. In the 16 cases in which adequate cytologic material was obtained, the cytologic diagnosis agreed with our favored clinical diagnosis in 13 of 16 patients (75%): [2 of 2 patients with suspected lymphangioma, in 6 of 6 patients with suspected metastasis, in 4 of 5 patients with suspected lymphoma, 0 of 2 with suspected orbital inflammation, 1 of 1 with suspected hemorrhagic vascular malformation]. Of the 2 cases with suspected inflammation, the cytologic diagnosis was metastasis in one case and epidermoid carcinoma of sinus origin in one case. One case of suspected lymphoma proved to be a metastasis. Immediate lid ecchymosis occurred in 3 cases (18%). Self-limited hemorrhagic proptosis was observed in one case (patient with vascular malformation). |
| Conclusie | NAB is a reliable diagnostic technique for suspected orbital lesions with a diagnostic accuracy of 94%. This method is mainly indicated in clinically suspected orbital lymphoma and metastasis. |
