DoelTo determine the predictive value of optic disc hemorrhages (ODHs) for the development of glaucoma.
MethodesThe European Glaucoma Prevention Study was designed to evaluate whether reducing the intra-ocular pressure in patients with ocular hypertension might reduce the risk of progression to glaucoma. All eyes included in the EGPS study were monitored for optic disc hemorrhages (ODHs) every 6 months for 5 years both by fundoscopy and evaluation of optic disc stereo-photographs. Resultaten1923 eyes of 1081 participants were analyzed. Thirty-three eyes (1.7%) developed ODHs on at least one occasion. The total percentage of eyes that developed glaucoma was 6,1% (117 out of 1923 eyes). Of patients that developed glaucoma, 18.2% (6 out of 33) had a previous ODH, whereas 5.9% (111 out of 1890) did not have a history of ODH (p=0.0127 by Fisher Exact test). Using a Cox Regression test with time-varying covariate, taking into account both the differences in follow-up and the moment of occurrence of the ODH between the patients, the hazard ratio for the first occurrence of ODH was 4.55 (p=0.001). Finally, to overcome the problem of clustering of the eyes within a patient, and the presence of interval-censoring, a hazard ratio was calculated for each of the 10.000 non-parametric bootstrap samples. The mean hazard ratio was still 3.97, although this relation was only borderline significant (p=0.06).
ConclusieThese results indicate that optic disc hemorrhages are associated with an increased risk of conversion to glaucoma.
