TitreDescemet’s Stripping Automated Endothelial Keratoplasty (DSAEK): Results of 45 consecutive cases

ButTo evaluate the visual, topographic, and endothelial cell count results in the first 45 consecutive patients who have received Descemet’s Stripping Automated Endothelial Keratoplasty (DSAEK) surgery in the Rotterdam Eye Hospital. Furthermore, some techniques to improve graft insertion and adherence will be discussed.
MéthodesSince December 2005, 45 patients underwent DSAEK surgery for Fuchs’ endothelial dystrophy or pseudophakic bullous keratopathy. Snellen visual acuity, refractive astigmatism, endothelial cell counts, and corneal topography were prospectively measured.
RésultatsBSCVA averaged 0.3 (range 0.01 to 0.5) preoperatively, 0.56 (range 0.16 to 1.0) at 3 months, and 0.64 (range 0.3 to 1.0) at 6 months follow-up (all data in terms of Snellen visual acuity, corrected for amblyopia and macular disease). At 6 months follow-up, topographic astigmatism was increased with 0.1 D and refractive astigmatism increased with 0.4 D, both compared to preop measurements. A slight hyperopic shift in refraction of 0.34 D was noted at 6 months follow-up, compared to preop. Mean endothelial cell count at 6 months follow-up was 1406 cells/mm2 (range 761 to 1846). At last follow-up, all grafts were clear and functional. No primary graft failures were noted.

ConclusionDSAEK surgery is a feasible technique, with considerable advantages over penetrating keratoplasty, in the treatment of endothelial dysfunction. The most frequent peroperative challenge is unfolding of the graft in the anterior chamber after insertion and obtaining adequate donor attachment.
