TitreUrrets-Zavalia syndrome after intraocular refractive surgery
ButTo discuss the possibility of Urrets-Zavalia syndrome after various types of intraocular refractive surgery
MéthodesThree eyes of three patients underwent uneventful refractive
surgery : one patient had a phakic intraocular lens implant (Artiflex) for
high myopia, another patient underwent clear lens extraction for high
myopia, and a third patient had a ICL phakic intraocular lens implant.
All three eyes developed high postoperative intraocular pressure with a
subsequent fixed dilated pupil (Urrets-Zavalia syndrome).
RésultatsResults Two out of three patients had a permanently fixed semi dilated pupil
and continued to complain about photophobia. Possible treatment options include
either a conservative approach by using soft coloured contact lenses, or more invasive
procedures. These would consist of removal of the phakic IOL,
subsequent phacoemulsification and use of large diameter rigid iris
diaphragms, overlapping interdigitating iris rings, capsular tension rings
with opaque iris segments, intracapsular Hermeking iris prosthetic system
implants, or custom iris implants with enclavation fixation
ConclusionUrrets-Zavalia syndrome is a rare but real complication even
after uneventful intraocular refractive surgery. Care must be given to
prevent potential intraocular pressure spikes in the immediate postoperative
period.
