TitrePars plana vitrectomy in uveitis patients

ButTo examine the causes and success rates of vitrectomy in uveitis patients.
MéthodesWe have retrospectively reviewed the charts of 28 uveitis patients (31 eyes) who have undergone vitrectomy in the last 4 years for other causes than reghmatogeneous retinal detachment. We have then looked for operation causes and success rates. Success rates were based on visual outcomes, complications, uveitis relapses and diagnosis in case of vitreous biopsy.
Résultats71% of eyes had therapeutical vitrectomy, 22% were operated for macular pucker and 7% had a diagnostic vitrectomy. In total laboratory tests were conducted in 51 %. Visual acuity improved in 19 eyes (61%), with a median improvement of 5 snellen lines, remained stable in eyes 4 (13%) and decreased in 5 (16%). Three eyes (10%) were lost for follow up. Post-operative complications were cystic macular oedema in 4 eyes (13%), cataract in 5 eyes (16%), retinal detachment in 3 eyes (10%) and glaucoma in one eye (3%). Ten eyes (32%) experienced at least one uveitis relapse. Diagnostic tests was performed in 16 eyes (51%) with a success rate of 50%.
ConclusionTherapeutical vitrectomy in patients with uveitis may improve visual acuity. However, the effect might be transient and operation should only be performed when the patient visual acuity is low despite adequate medical therapy. Macular peeling appears to have a good success rate in uveitis patients. Since laboratory tests are successful in only 50 % of cases, and considering the possible complications, diagnostic vitrectomy should be limited to selected cases.
