| Méthodes | Retrospective study of combined PDT and IVT-A in 21 patients (21 eyes). Eight eyes had initial retinochoroidal anastomosis (RCA) associated with pigment epithelium detachment (PED), 9 eyes had classic or predominantly classic choroidal new vessels (CNV) and 4 eyes had occult CNV. In the two last subgroups, the combined treatment was proposed after non responsive initial treatment by PDT alone. IVT-A using 4 mg of Kenacort in 0.1 ml was performed 8 days before PDT. Visual outcome and evolution of the exudative components were assessed on the basis of clinical examination, optical coherence tomography and fluorescein or ICG angiography. Potential complications including ocular hypertony, cataract and endophthalmitis were recorded. |
| Résultats | Mean follow-up was 6.3 months. In group 1 (RCA), visual acuity improved in 6/8 eyes (75%) with complete regression of the exudative components and PED. In group 2 (classic CNV), stable or improved visual acuity was found in 6/9 eyes (66%) with regression of the exudative signs. In 3 of those patients, CNV was replaced by a fibro-glial tissue. In group 3 (occult CNV), only 1/4 eye (25%) retained the same visual acuity and showed disappearance of fluid. Ocular complications following IVT-A was restricted to ocular hypertony in 3 eyes (14%), successfully managed by topical medication. |
| Conclusion | In this small series, combination of IVT-A with PDT seems to have achieved some beneficial effects in the management of exudative AMD, especially in the RCA group. Randomized studies are needed to assess this benefit. |
