| But | To show the management of a case with Multiplex Sclerosis who was to be operated for cortisonic cataract. The patient was scheduled under topical anesthesia. Posterior capsulorhexis was needed to avoid later YAG capsulotomy as the patient was poorly amenable to examinations and treatments. |
| Méthodes | Agitation, with eye and head large movement resulted in a dangerous procedure. Posterior rhexis could not be considered anymore. I took the decision to ask a transformation towards general anesthesia in the middle of the operation. |
| Résultats | Under general anesthesia, cataract extraction, posterior rhexis and implantation were easy. Intra-operative conversion presented no difficulty. |
| Conclusion | It can be advisable to switch from topical to general anesthesia to ensure a safe surgery, specially if delicate manoeuvres suppose to be performed. |
