ButTo report two cases of Birdshot chorioretinopathy with an atypical presentation.
MéthodesBirdshot chorioretinopathy (BC) is characterized by vitritis without snowbanking and typical subretinal cream coloured spots.
We report two cases of an atypical presentation. The first case is a Caucasian 40-year-old male who was followed for almost two years for a bilateral intermediate uveitis without any fundus lesions. Second case is a Caucasian 59-year-old male referred to us with a bilateral intermediate uveïtis. General work-up showed a disturbed liver function. An autoimmune hepatitis was suspected.
RésultatsIn both cases vitritis with snowballs was noted. Fluorescein angiogram (FA) showed no particular lesions. HLA typing demonstrated the presence of HLA-A29. Indocyanine Green Angiography (ICGA) was performed, showing the typical BC lesions.
The hepatitis of the second case turned out to be toxic. An extensive work-up showed no other abnormalities in both cases.
ConclusionDuring 2 years, both cases initially showed an intermediate uveitis with no typical fundus lesions. HLA typing showed a HLA-A29 positivity and ICGA revealed typical dark spots corresponding to BC, although fundus and FA were negative.
In the work-up of intermediate uveitis, Birdshot chorioretinopathy should be ruled out.
