Titre5-Year retrospective experience with non-penetrating deep sclerectomy

ButTo assess long-term safety and effectiveness of non-penetrating deep sclerectomy (NPDS).
MéthodesRetrospective non randomized study including 234 eyes (179 patients) with medically uncontrolled glaucoma.42 eyes had previous filtering surgery,113 had severe glaucomatous damage.Procedures were performed according to Kozlov-Mermoud’s technique and associated with SKGEL® or TFLUX® implant in 65 eyes, antimetabolite augmentation (AMETAB) in 36 and combined implant-AMETAB in 124.
RésultatsMean follow-up was 32.0+/-19 months. 1st month postoperative complications were observed in 144 eyes with hyperfiltration in 38, scarring of filtration bleb in 49, iris incarceration in 14. 5-fluorouracile injections were given in 76 eyes.Yag goniopuncture was done in 142 eyes, complicated by iris incarceration in 7. Late spontaneous iris incarceration occurred in 4 eyes. Mean visual acuity was 0.7 preoperatively and 0.6 at the last visit(p<0.05). Mean IOP was decreased from 28+/-9mmHg to 16+/-6mmHg (p<0.05)and stable throughout the follow-up.Mean number of medications was reduced from 2 to 1.2. Complete (target IOP reached without med) and qualified (target IOP reached with and without med) final success rates were 39% and 72%. 50% achieved a final IOP<15 mmHg.Complete and qualified success probability was 56% and 78% at 36 months and 24% and 50% at 60 months. Final IOP, success rates and probability was comparable with or without AMETAB.Success probability was higher in the subgroup without implant(p<0.05).
ConclusionNPDS was a relatively safe and effective procedure.Yag goniopuncture and adjunctive medications were frequently needed.AMETAB and placement of implant did not seem to improve success rate.Iris incarceration must be monitored
