TitleDry Eye Syndrome – New insights into the Diagnosis in Major Collagenosis

PurposeTo diagnose the patients with ocular Sicca syndrome in major collagenosis and to evaluate the clinical and histopathological lesions, related to stage of the disease, we tried to establish significant correlations between clinical tests for lacrimal tear and the severity of ocular disorders.
MethodsWe have evaluated all patients with major collagenosis, in the last 5 years: rheumatoid arthritis (211 cases), systemic lupus erythematosus (106 cases), scleroderma (54 cases). General and ocular examinations have been performed, during hospitalization in ophthalmological or internal disease departements, to all patients with Sicca syndrome.
Results371 patients have been included in this trial, with a follow-up period of 18,6 months. The diagnosis of Gougerot-Sjögren syndrome was present in 44 cases. The ophthalmological exam consisted in tests of lacrimal evaluation (test Schirmer I, Jones, BUT, tear crystalization), and a methodic biomicroscopic examination of ocular surface without and with vital staining. The global test of lacrimal evaluation recorded low values in Sicca syndrome cases, revealing a correspondence between the results and the appearance of corneal complications. Type III and IV crystalization occurs in 87% of the patients with Sicca syndrome in harmony with the dry eye symptoms and the objective tests of examinating the lacrimal film and the ocular surface. The cellular anomalies, in different associations and intensities were present in patients with Sicca syndrome type I-IV.
ConclusionThe ophtalmological exam is necessary to all the patients with collagenosis who present ocular symptoms. RA is the colagenosis which most frequenly associates ocular Sicca syndrome in mild and medium clinical forms.
