TitleVitrectomy for premacular haemorrhagic cyst in shaken baby syndrome.
PurposeTo discuss the surgical treatment of premacular haemorrhagic cysts in shaken baby syndrome.
MethodsCase report and review of the literature.
ResultsA 4-month-old female infant with shaken baby syndrome presented with bilateral premacular haemorrhages. Fixation of light was unstable and nystagmus was present. Because of the high risk of amblyopia due to visual deprivation, vitrectomy was performed bilaterally. After posterior vitreous membrane detachment and drainage of the premacular haemorrhage, the premacular membrane was excised in the area of the cyst. During left eye surgery whitening of the macula (as in Purtscher retinopathy), and a tuft of new vessels with fibrous proliferation on the optic disc were observed. Two months after surgery the infant demonstrated visual attention, both retinas were attached, but the left optic disc was pale, suggesting optic atrophy.
ConclusionVitrectomy for premacular haemorrhagic cysts in infants can offer visual rehabilitation especially when both eyes are affected. Retinal ischemia in a confined space as a possible cause of the observed disc new vessels is hypothesized.
