TitleComparing the treatment efficacy and safety of the Paul glaucoma implant and the Baerveldt glaucoma implant
PurposeTo compare the efficacy and safety of the Paul glaucoma implant (PGI) and the Baerveldt glaucoma implant (BGI) in the treatment of glaucoma, during a follow-up period of twelve months.
MethodsIn this retrospective study 23 patients treated with the PGI and 27 patients treated with the BGI were followed up during a period of twelve months postoperatively. The primary outcome was failure defined as intraocular pressure (IOP) >18mmHg and <=5 mmHg or less than a 20% reduction in IOP from baseline, the need for additional glaucoma surgery or loss of light perception vision.
ResultsAt one year postoperatively the cumulative failure rate was similar for both groups, 27.3% (95% CI (6.1% -43.7%)) for the PGI group and 28% (95% CI (8.1%-43.6%)) for the BGI group. The mean IOP in the PGI group decreased from 23.7+-6.9 mmHg preoperatively to 13.1+-2.9 mmHg at one year postoperatively. In the BGI group the IOP was reduced from 26.0 +-7.3mmHg at baseline to 10.44.9mmHg at one year. Only at one week and one month after surgery the IOP was significantly lower in the PGI group, 13.6+-6.1 mmHg for PGI versus 20.1+-7.4 mmHg for BGI (P<0.0025) and 14.6+-3.8 mmHg for PGI and 21.25.8 mmHg for BGI (P<0.0025) respectively. The two groups had similar complication rates, with a trend to more, especially early, complications in the BGI group.
ConclusionBoth PGI and BGI are safe and effective in reducing the IOP and the need for glaucoma medication. The BGI group showed an early phase of relatively elevated IOP with no significant difference in safety or efficacy measures at twelve months.
