TitlePeripapillary intrachoroidal cavitation at the crossroads of myopic peripapillary changes
PurposeTo establish if peripapillary intra-choroidal cavitation (PICC) correlates with gamma peripapillary atrophy (γPPA) presence in the absence of peripapillary staphyloma (PPS). In addition, to analyze the border tissue (BT) discontinuity in PICC.
MethodsThis prospective cross-sectional non interventional study included highly myopic eyes. Non-highly myopic eyes were used as control. Radial and linear scans centered on the optic nerve head were performed using Spectral-Domain Optical Coherence Tomography (OCT). OCT variables were analyzed along the twelve hourly sections, in both eyes of each subject. The Chi-squared test of independence and Fisher's exact test were used for statistical analysis.
ResultsA total of 667 eyes from 334 subjects, including 203 (60%) women, were included. The mean age was 49.7 ± 18.0 years. Two hundred twenty-nine eyes were highly myopic. PICC was found in 40 (13.2 %) of highly myopic eyes. PPS and γPPA co-existed in 136 eyes and 22.7% of these eyes had PICC. All PICC eyes showed with this co-existence and also discontinuity of the BT in the γPPA area
ConclusionPICC is found solely in eyes combining PPS and γPPA. However, only 22.7% of these eyes exhibit a PICC, suggesting the role of other factors. We confirm the exclusive presence of BT discontinuity in all PICC eyes. This finding suggests the involvement of mechanical factors in the genesis of PICCs. Longitudinal and biomechanical studies will specify the mechanisms involved.
